You will need to create a username and password to log-in to the patient portal, complete the patient form(s), and submit them electronically.  Completing the forms prior to your visit is mandatory and will help expedite the new patient registration process.

Also, be sure to bring any diagnostic tests, prior psychiatric or psychological evaluations, lab results, your medications, and any additional information that you believe will be helpful in conducting a thorough evaluation.
